Neighbouring groups of GP practices across the country – typically covering 30-50,000 people – are funded to work together to improve population health and extend the range of convenient local services, creating genuinely joined-up teams of general practice, community health and social care staff, as well as other local organisations.
These new primary care networks (PCNS) – or primary care homes (PCHs) as we call them locally in Wakefield – will provide flexible options for neighbouring groups of GP practices, wider primary and community care teams, local organisations and local people to plan and deliver better health for everyone living in their local communities.
GP practices across Wakefield are expected to formally announce their PCH arrangements by July 2019. As Wakefield’s GP Confederation, Conexus Healthcare are supporting Wakefield’s PCHs as they set up, develop and grow.
