Getting started with care navigation?

We can help your practice, primary care network or area introduce care navigation for the first time or as a relaunch.

We know the GP isn’t always the best person to see. Patients can often be seen and treated quicker by a nurse, clinical pharmacist, or other members of the wider practice team and in some cases, the GP practice might not be the right place at all. That’s where ‘active signposting’ or care navigation as we call it comes in.

Care navigators are receptionists and admin staff put through specialist training. They also possess additional skills to provide a crucial practice role in improving access to primary care. All to help patients to get to the right service faster.

Here's how our approach works:

Step 1: Reviewing the Foundations

Our expert consultants will help you take stock and assess your current position in relation to active signposting and care navigation. This short, structured session will help you listen to the views of stakeholders, clearly identify challenges and priorities, and engage local leaders in agreeing next steps.

Step 2: Designing the Local Model

An online workshop with stakeholders to:

map existing services and access pathways.
confirm the services that care navigators can signpost to across your area.
sign-off the signposting criteria to these services.
develop a local system template.

Step 3: Building your Care Navigation Template

Our Business Intelligence team work with your area to create a template for you to embed into SystmOne or EMIS. This bespoke template will contain all the internal and external navigations agreed in Step 2 and be used by staff daily to record patient signposting.

Step 4: Care navigation Training for Frontline Staff

Your reception/admin teams will complete our endorsed e-learning to understand the key principles of care navigation. Staff receive a certificate upon completion before partaking in an interactive session that covers how to make a navigation more likely to succeed, an introduction to the services in your template and a run through of how to use it.

Refresher training for care navigators

Delivered online or face-to-face, this training is perfect for new starters or care navigators who need to refresh their knowledge. Together, we’ll review what care navigation is, learn how it is done locally, revisit the system template and understand more about the services we can signpost to.

Practice visits

Available online or face-to-face, this is a unique chance to review how the introduction to care navigation is going in a particular practice, identify barriers and facilitate problem-solving. This will be bespoke to your practices needs and can cover any particular area you need.

Super user group

An interactive session designed to bring together a group of care navigators from across practices in your area. The aim: to start to develop a greater depth of knowledge and to help you 'grow your own' local experts. These experts will become your signposting champions and can even go on to train new reception staff in your area. Tailored to local need.
