This is our privacy notice. It tells you how we collect and process your data when you use our website: conexus-healthcare.org.

Our website and services are not intended for children under 13, and we do not knowingly collect personal data from them. Personal data

We get personal information about you when:

  • you contact us about our services through a contact form on our website, by telephone, email or otherwise;
  • you contact us and give us your personal information through social media sites such as Facebook or LinkedIn.

The types of personal information we might store include your name, the organisation you work for, your telephone number and your email address.

Technical and usage data

This may include your internet protocol (IP) address, browser type and version, browser plug-in types and versions, time zone setting and location, operating system and platform, and other technology on the devices you use to access this site.

Our website uses cookies. Cookies are text files placed on your computer to collect standard internet log information and visitor information. These cookies allow us to distinguish you from other users of the website and allow us to collect information about your visit to our website.   • to perform the contract, or supply you with the products or services you have asked for. The basis for this processing is to perform the contract with you related to these activities and services, or because you have asked us to take specific steps before entering into a contract in respect of these activities and services;

  • to contact you with news of our organisation and information about the work we do. This processing is necessary for the administration of our business and the provision of our services, which is necessary for the legitimate interests of our business;
  • to provide you, or permit selected third parties to provide you, with information about goods or services we feel may interest you. Change of purpose

5. Your legal rights

You have the right to ask for a copy of the information that we have about you (a Subject Access Request) and you can ask us to correct any incorrect data we have about you too. You can also:

  • request erasure of your personal data;
  • object to processing of your personal data;
  • request restriction of processing your personal data;
  • request transfer of your personal data;
  • withdraw consent;
  • ask us to stop contacting you with direct marketing.

You will not have to pay a fee to access your personal data (or to exercise any of the other rights). However, we may charge a small fee if your request is clearly unfounded, repetitive or excessive. We try to respond to all legitimate requests within one month. Occasionally it may take us longer if your request is particularly complex or you have made a number of requests; in this case, we will notify you and keep you updated. Alternatively, we may refuse to comply with your request in these circumstances.

6. How long we keep your personal data

We will only retain your personal data for as long as necessary to fulfil the purposes we collected it for, including for the purposes of satisfying any legal, accounting or reporting requirements. Details of retention periods are available in the NHS Records Management Code of Practice.

7. Data security

We have put in place appropriate security measures to prevent your personal data from being accidentally lost, used, accessed, altered or disclosed in an unauthorised way. We limit access to your personal data to those who have a genuine business need to know it, and we have procedures in place to deal with any suspected data security breach. We will notify you and any applicable regulator of a suspected breach where we are legally required to do so.

8. International transfers

Some of our third-party service providers may process your personal data outside the UK. Where this happens, we ensure a similar degree of protection by using transfer mechanisms approved under UK data protection law, such as the UK International Data Transfer Agreement or the UK Addendum to the EU Standard Contractual Clauses.

9. Making a complaint about how we use your personal data

If you are unhappy with the way we have handled your personal data, you have the right to make a complaint to us. We take all data protection complaints seriously and have a process in place to deal with them. To help us deal with your complaint efficiently, please include your name and contact details, a description of your concern, and any relevant dates or reference numbers.

When we receive your complaint, we will acknowledge it within 30 days. We will then investigate your concerns, take appropriate steps to address them, and inform you of the outcome of your complaint without undue delay.
