Our location
For roles based at Conexus HQ, our office is at Sandal Castle Medical Centre, located in the suburbs of Wakefield, just five minutes off the M1.
Perfectly positioned on a retail estate joined with Asda, Aldi, cafés, pubs and restaurants, the office has good transport links and free parking.
As a team, we capitalise on the local amenities and walking routes. We frequently catch-up as a group around the lakeside at Pugney’s, or grab a cuppa in one of the local cafés or pubs.

Nursing Associate
Job summary
To support the Primary Care Network team in the delivery of nursing services, working as part of the PCN multi-disciplinary team to complement the existing workforce. This role will provide nursing support to the PCN Nurse (Clinical Care Co-ordiantor), Practice Nurses and General Practitioners to ensure provision of holistic and person-centred care of the full range of nursing services to the entitled patient population.
This role will assist clinical staff across the PCN in the provision of treatment, preventative care and health promotion. You will work as a member of the Nursing team, under the guidance of a Registered General Nurse, and must work at all times within your own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).
You will be responsible for the care delivered, focusing on supporting patients to be healthy, monitoring of long-term conditions, health prevention and education advice and screening activities. You will work collaboratively to meet the needs of patients, ensuring the highest standards of care and supporting the delivery of policy and procedures.

Primary Care Workforce Lead
Job summary
Conexus hosts staffing for all 7 of Wakefield Districts Primary Care Networks (PCNs) which are an amalgamate of the 34 practices in the district. We provide support to our practices in a number of ways but support with HR and workforce planning is a key component.
We currently employ around 130 staff through the Additional Roles Reimbursement Scheme (ARRS) and have an ambitious plan across the district to increase these roles over the coming 12 months. As part of the role, the incumbent will support our PCNs and Conexus to plan, recruit and retain staff in these areas either through direct employment in Conexus or through sub-contracts. This will include supporting the Conexus Senior Leadership Team in developing a competency assurance approach through professional supervision.
The individual will also support wider General Practice in the district to maximise our recruitment and retention of staff, linking to wider partnership schemes within West Yorkshire Integrated Care System.
Lastly, the post-holder will provide HR advice and guidance concerning the Conexus employed staff.

Pharmacy Technician
Job summary

Clinical Pharmacist
Job summary

Pharmacy Technician
Job summary
We are West Wakefield Primary Care Network and we are developing a wide range of services to support general practices and improve outcomes for patients across the West of Wakefield.
We would like you to join our excellent and supportive team as a Pharmacy Technician to provide safe, quality, and cost-effective pharmacy services in primary care working with a wide range of colleagues within the Primary Care Network (e.g. Care Co-ordinators, Health and Wellbeing Coaches, Social Prescribing Link Workers, Nurse Associate, Advanced Nurse Practitioners, Occupational Therapists); alongside the wider team including the Practices, Community Pharmacy, the Voluntary and Community Sector, as well as statutory service providers including the CCG.
Our aim is to create a diverse team that support one another, have opportunities to share learning and engage in professional development, while being integrated into the practices. You will work closely with the existing clinical teams to deliver excellent patient care working innovatively and collaboratively with colleagues.

Nursing Associate
Job summary
We are West Wakefield Primary Care Network and we are developing a wide range of services to support general practices and patients in our PCN across the Wakefield district.
We would like you to come and join our Nursing Associate team providing a mix of practice (the majority of the time) and community based services to patients including housebound, care homes, and CVD outreach. You will join a supportive team, have the opportunity to work independently under the leadership of registered nurses, and have lots of opportunities to undertake training and develop your role so that you can provide a comprehensive service with a broad range of skills. Ideally you will have completed your Nursing Associate training and be registered with the NMC.
We will provide a supportive learning environment, especially structured in the first preceptorship year. We will help you set out and revise specific educational goals and ensure you receive appropriate clinical supervision, you will receive regular appraisals. You will be supported to attend agreed CPD.
We are committed to improving the care provided to patients, reducing health inequalities and raising the quality and standards of GP practice. This role will enable you to deliver person-centred co-ordinated care, and support implementation of the NHS long-term plan.
