Our location

For roles based at Conexus HQ, our office is at Sandal Castle Medical Centre, located in the suburbs of Wakefield, just five minutes off the M1.

Perfectly positioned on a retail estate joined with Asda, Aldi, cafés, pubs and restaurants, the office has good transport links and free parking.

Since moving to the office in July 2019, our team has worked to put their stamp on it. The open plan office is a calm environment, with space to breakout and collaborate with colleagues. With colleagues we combine this with supportive hybrid working where staff work effectively as a member of the team from home. Many staff are based at practice sites across the District.

As a team, we capitalise on the local amenities and walking routes. We frequently catch-up as a group around the lakeside at Pugney’s, or grab a cuppa in one of the local cafés or pubs.

Practice Nurse (Roving Team)

Job summary:
An exciting opportunity has arisen for the employment of a Roving Practice Nurse to join Conexus Healthcare. As a Practice Nurse you will be working within Trinity Health Group Primary Care Network (PCN).

The Roving Practice Nurse will provide and maintain a high standard of nursing care for patients with a particular focus on those who are housebound or hard to reach. Focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the General Practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Business Support Administrator

Job summary:

The purpose of this role is to provide comprehensive high-quality administration and co-ordination support to deliver an effective service.

As a highly motivated individual, the post holder will be expected to provide high quality administrative and practical support across the responsibilities of the Covid-19 Testing Service. This is a fast-paced environment that requires excellent organisational skills.

Utilising excellent communication skills, the Business Support Administrator will develop professional working relationships with members of Conexus Healthcare and external stakeholders including workers within the service, our membership practices, the local authority, clinical commissioning group, local health trusts and other providers.

Advanced Clinical Practitioner

Job summary:

This is an exciting opportunity to join West Wakefield Primary Care Network as an Advanced Clinical Practitioner.

Conexus Healthcare Ltd. is a GP Confederation in Wakefield. In addition to national training, and local clinical services, Conexus provides support to local PCNs by hosting staff and supplying services such as HR and accountancy. The successful candidate will be an employee of Conexus Healthcare Ltd. However, their work will be practice-based under the direction of the clinical director(s) and the host practices.

You will be working across all practices within the PCN, as part of the PCN Team. There is an expectation of autonomous working, however, collaboration within practices and the wider PCN team is a key part of the role. The aim being to provide patient-focused care responsive to health need.

Job summary:

Pharmacy Technician

Job summary:

West Wakefield is a Primary Care Network of 5 practices; the team at West Wakefield Primary Care Network are a network of forward-thinking General Practices covering circa 68,000 patients in the West Wakefield area. Our aim is to create a diverse team that support one another, have opportunities to share learning and engage in professional development, while being integrated into the practices. The successful applicant will work closely with the existing clinical teams to deliver excellent patient care working innovatively and collaboratively with colleagues.

Job applicants should be aware of the guidance from NHS England and NHS Improvement, that a formal announcement has been made that individuals undertaking CQC regulated activities in England must be fully vaccinated against Covid 19 by no later than 1 April 2022 to protect patients. These regulations are expected to come into force from 1 April 2022 and therefore, those who are unvaccinated are encouraged to ensure that they have had their first does of the vaccination by 3 February 2022 in order to ensure they have received their second does by 1 April 2022. There are specific exemptions includes those who are medically exempt. Care Coordinator (Wellbeing Team)

Job summary:

The post holder will be responsible for consulting with patients and determining their needs, developing care plans, co-ordinating patient care services, educating them about their condition, and working with the care team to evaluate interventions. You will need to display a compassionate nature, be knowledgeable about health care practices, and provide exceptional customer service. The role would be ideal for someone with proven experience as primary care healthcare assistant. You would be joining the existing well-being team made up of existing care coordinators employed by the PCN, and a social prescriber (currently Live Well Wkefield). The team is quite new, yet they have still managed to build a supportive and well-connected team despite working through Covid restrictions. The team are looking forward to developing their roles and seeing their team grow. The Care Coordinators have become an asset to the PCN and the practices/patients that they serve.

Primary Care Network Physician Associate

Job summary:

This is an exciting opportunity to join Brigantes as a Physician Associate to enable the Primary Care Network.

To be responsible for the provision of a clinical service, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The physician associate will be required to work autonomously in the practice’s, providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure when required.
