Our location

For roles based at Conexus HQ, our office is at Sandal Castle Medical Centre, located in the suburbs of Wakefield, just five minutes off the M1.

Perfectly positioned on a retail estate joined with Asda, Aldi, cafés, pubs and restaurants, the office has good transport links and free parking.

As a team, we capitalise on the local amenities and walking routes. We frequently catch-up as a group around the lakeside at Pugney’s, or grab a cuppa in one of the local cafés or pubs.

Advanced Clinical Practitioner

Job summary

Are you working at an advanced level assessing and treating all ages of patients with undifferentiated conditions and are looking for a change? Here at Brigantes Primary Care Network we are looking for you! We need you to join our expanding multidisciplinary team as an Advanced Clinical Practitioner.

Within your professional boundaries you will be working across all practices within the PCN to assess and diagnose. There is an expectation of autonomous working, however, collaboration within practices and the wider PCN team is a key part of the role. The aim being to provide patient-focused care responsive to health need. This is an ideal role for you looking for a better work-life balance as you will working practice hours Monday to Friday.

Primary Care Network Physician Associate

Job summary

We are Brigantes Primary Care Network and we are looking for you to join our expanding multidisciplinary team to help develop integrated care and fulfil the requirements of the PCN objectives. We currently have a team of two Physicians Associates, we are hoping to make you our third!

You will be responsible for the delivering of patient-centred care. You will be required to work autonomously in the practices, providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure when required.

Long Term Condition Home Visiting General Practice Assistant (GPA)

Job summary

The Long Term Condition Home Visiting General Practice Assistant (GPA) will join a team within Five Towns PCN to support the care of house bound patients. With the support of the team the role will provide initial assessments and planning for patients with a holistic approach. Ultimately the aim is to improve the patients access/experience in practice.

PCN Business Support Officer

Job summary

In line with our third area of work, detailed below, the purpose of the role will be to provide high quality administration and project support to the Primary Care Networks (PCNs) in Wakefield.

As a highly motivated individual, the post holder will be expected to lead on the provision of high quality general, financial, HR and corporate administrative support across multiple PCNs. Managing the design, implementation and ongoing development of systems, processes and policies for the smooth running of all back-office functions.

Utilising excellent communication skills, the Business Support Officer will develop professional working relationships with members of: Conexus Healthcare Ltd, staff in the Primary Care Networks and other system partners.

The post holder will work from multiple sites across Wakefield & District and will work flexibly to meet the needs of the networks.

Job summary

Trinity Health Group PCN are looking for a Paramedic to join our growing ARRS team. You will work in partnership with the practice's, as well as the wider PCN team in developing this role further. Are you currently working or willing to work towards a relevant level 7 or equivalent academic qualification? Do you have experience of working with a multidisciplinary team to improve patient care? Is working in Primary Care something you have always wanted to do?

Clinical Pharmacist

Job summary

This is an exciting opportunity to join our team as an enthusiastic and forward thinking Clinical Pharmacist, working with general practices across our PCN footprint, with support from our existing clinical teams to deliver excellent patient care. Based at Tieve Tara Medical Centre this role will involve working across multiple sites within the PCN. We have an established team of Clinical Pharmacists and Pharmacy Technicians, receiving support from a Senior Pharmacist and GPs who are able to develop and mentor our team.

The role includes focusing on elements of the Impact and Investment Fund (IIF) and supporting elements of PCN Contracts which our GP practices follow.

Clinical Pharmacist

Job summary

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and reauthorisation, acute prescription request, while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN. The post holder will perform face to face medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple comorbidities. The post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice. The post holder will be supported to develop their role to become a non-medical prescriber, if that qualification is not already held.
