Celebrating nine years of Team Conexus

Nine years ago, Conexus began with just two people and a vision to support healthcare across the Wakefield district. Today, we’re celebrating our anniversary as a nearly 500 person-strong workforce, and the journey has been nothing short of remarkable. As we reflect on how far we’ve come since 2017, we’re filled with pride for what we’ve accomplished together and anticipation for what lies ahead.

Our Team

Every member of our organisation plays a vital role in our success. Whether you’re part of the core Conexus team, working within our GP Care Sessional workforce, or delivering care through our Primary Care Networks, you help us make a tangible difference in the lives of people across all of the Wakefield district.

Your expertise, compassion, and commitment are what drive our impact every single day.

Our Partnership Model

We are a GP Federation, owned by and accountable to our local GP practices, which means everything we do is designed to support General Practice as the “neighbourhood anchor” for health in Wakefield. This partnership model ensures we remain grounded in the real needs of our community and responsive to the challenges facing our member GP Practices and primary care.

This collaborative approach ensures patients receive the right care, in the right setting, at the right time. By working closely with our GP Practices and partner organisations across the district, we’re delivering healthcare services and building a stronger, more resilient, and healthier communities for everyone who lives in the Wakefield district.

Looking ahead: Our 2026–2031 vision

As we enter our tenth year, we’re launching an ambitious Corporate Strategy focused on “restoring and strengthening generalism”. Our goal is to lead the way in reducing unnecessary administrative burden on General Practice while improving patient care and supporting our workforce to do what they do best.

Our five-year priorities include:

  • Leading neighbourhood health teams to focus on prevention and address health inequalities across our communities

  • Improving coordination between primary and secondary care to eliminate duplication and reduce administrative delays that impact both staff and patients

  • Investing in digital capabilities and AI tools to streamline workflows and make everyone’s working lives easier

  • Pursuing Investors in People accreditation to ensure Conexus remains an exceptional workplace where talent thrives

These aspirations represent our commitment to practical innovation that will make a real difference to both our teams and the people we serve.
