Book appointments at evenings and weekends

Same day and routine evening/weekend appointments are provided by Conexus on behalf of NHS Wakefield CCG. The service is known locally as GP Care Wakefield.

Most practice telephone systems automatically divert to the service when closed so if you’re a local practice, please encourage your patients to call your practice telephone number for help during these times.

Patient call-backs during the week can be booked from 3:30pm onwards – simply book into the next available slot, using the process within the GP Care Wakefield Reception Manual.

Using the evening and weekend clinic is a faster way for patients to get help and speak to a GP when suddenly unwell at evenings and weekends. In most cases they might not even need to leave home. Most problems can be dealt with over the phone and recorded straight in to the patient’s record, with any urgent tasks flagged for you."

Appointments during Coronavirus

To help protect patients and our clinical teams during coronavirus, most appointments are being carried out over the telephone or by video call. Patients will only be asked to come in for a face-to-face appointment if it is deemed clinically necessary. Routine nurse appointments are slowly being re-introduced. How GP Care Wakefield works

Your practice team can book directly into GP Care Wakefield using SystmOne or the patient’s call is diverted at evenings & weekends if they call your practice telephone number. Patients contacting 111 may also be referred to GP Care – we will receive a task from 111.

All calls are answered by a friendly care navigator who works here in Wakefield, usually within a few minutes.

Patients are encouraged to explain their symptoms as clearly as possible so the care navigator can book the patient into the relevant clinic list or advise them to contact their practice if more appropriate.

GP Care Wakefield opening times

The GP Care phone lines are open:

Monday to Friday: 6pm – 10pm
Saturday/Sundays/Bank Holidays: 9am – 3pm

Wakefield GP practices can make direct referrals before the phone lines open on a weekday so there may be times when you are offered a consultation time before 6pm. We also receive referrals from NHS 111 and other urgent care services after the phone lines are closed on a weekend and we will offer consultations after 3pm on a Saturday or Sunday.

Our evening and weekend clinics are available by appointment-only. Face-to-face appointments are available at Trinity Medical Centre, Pontefract Hospital or Elizabeth Court Surgery.

Routine appointments at evenings and weekends

You can offer patients an evening or weekend nurse appointment at GP Care Wakefield if it suits them better.

These clinics are extremely convenient for patients and help your practice to improve access.

Appointments offered:

  • Annual reviews for heart disease, diabetes, COPD, asthma and stroke
  • Blood tests
  • Routine ECG (not chest pain)
  • Spirometry
  • Review of contraception pill
  • Repeat depo injection
  • Smears
  • B12 injection
  • Removal of sutures and clips

The service is not available for routine matters such as repeat prescriptions.
