Future SELPH

Every young person deserves the tools and support to feel good and live a vibrant life. No matter what! Future SELPH is commissioned by Conexus Healthcare and proudly delivered by voluntary community sector youth organisations with years of experience of working with young people.

‘Freedom to speak up’ guardian

2020 saw our development of the Wakefield District practices ‘Freedom to speak up’ guardian service.

It provides reassurance to general practice staff through a confidential service outside their own practice but embedded in primary care.

Conexus supports staff to highlight issues, concerns, or worries they have observed but may not feel confident sharing within their practice. We advise, support and guide those individuals, ensuring that patient care is prioritised.

Women's Health

We have begun to support a number of projects that support women's health. We are also developing a resource site of approved and updated information relating to women's health and women's services. More information will appear here soon.

Children's Observation Hub

The Children's Observation Hub is an enhanced access service for poorly children in Wakefield. The hub acts as a primary care based option for parents who might otherwise take their ill child to A&E, the hospital or phone 111. It offers late afternoon, evening and weekend appointments and can be accessed by phoning your usual GP Practice. If your child is ill, you should still contact your GP practice and they can refer you to the Hub. If your practice is closed, you will be diverted to GP Care Wakefield who can also refer you to the hub if needed.

Digital workforce scheduling

In light of the difficulties experienced by practices to organise the delivery of the 1st and 2nd COVID-19 vaccines, we wanted to support practices to adopt a more efficient and innovative way of administrating the upcoming COVID-19 booster and flu vaccinations.

Conexus teamed up with Doc Abode, who provide a digital workforce platform that allows practices and primary care networks in the Wakefield District to coordinate delivery of the vaccination programme for housebound patients by using a secure, flexible and integrated App.

With an initial implementation with colleagues at Health Care First, we were able to determine the benefits of using the App and then make this available to all GP practices in the District. This has successfully reduced 10 days of administration time into one click.
