Getting started with care navigation?

We can help your practice, primary care network or area introduce care navigation for the first time or as a relaunch.

We know the GP isn’t always the person that a patient needs to see. Patients can often be seen and treated more quickly by a nurse, clinical pharmacist, or other members of the wider practice team.  In some cases, the GP practice might not be the right place at all. That’s where ‘active signposting’ or care navigation, as we call, it comes in.

Care navigators are specially trained receptionists and admin staff. They also possess additional skills to provide a crucial practice role in improving access to primary care. All to help patients to get to the right service faster.

Further develop care navigation:

Refresher training for care navigators

Delivered online or face-to-face, this training is perfect for new starters or care navigators who need to refresh their knowledge. Together, we’ll review what care navigation is, learn how it is done locally, revisit the system template and understand more about the services we can signpost to.

Practice visits

Available online or face-to-face, this is a unique chance to review how the introduction to care navigation is going in a particular practice, identify barriers and facilitate problem-solving. This will be bespoke to your practices needs and can cover any particular area you need.

Super user group

An interactive session designed to bring together a group of care navigators from across practices in your area. The aim: to start to develop a greater depth of knowledge and to help you ‘grow your own’ local experts. These experts will become your signposting champions and can even go on to train new reception staff in your area. Tailored to local need.

Care navigation consultancy

In need of some general advice and support with your care navigation approach? Our trainers have implemented care navigation in their own practices and understand the various considerations that go into rolling out and maintaining our approach. They are on hand to discuss all your needs.
