Care Navigators – Weight Management and Physical Activity

Wakefield Council run an exercise referral scheme called Get Active. It’s for people that want to increase their physical activity to help manage their weight or health condition.

Service information

The service can help to:

  • Increase physical activity and fitness levels for people with preventable conditions
  • Compliment weight loss
  • Improve mental illness and mood
  • Reduce social isolation and improve lifestyle
  • Increase strength and mobility
  • Reduce falls


If the service is unable to help, the team can signpost people to other services that may be more appropriate for them.

How to access the service

The service is open Monday to Friday 8.00am till 5.30pm.

Your patient can self-refer by calling direct or if the patient is in the surgery already seeing the doctor or nurse, you can pass them a Get Active form for them to sign and send in to the service. When your patient calls up, the team shall talk through their referral. The team shall then arrange an appointment for the patient to meet one of the Health & Wellbeing Activators who will get them started on the scheme.
