Developing social prescribing in our communities is something we are hugely passionate about here at Conexus. In August 2017, our GP federation colleagues from West Wakefield Health & Wellbeing launched a unique social prescribing tool that was endorsed by NICE.
Together with partners in the Live Well Team from South West Yorkshire NHS Foundation Trust, the social prescribing tool consists of five sections based on the New Economics Foundation (NEF) Five Ways to Wellbeing and each section contains five questions that support the recommendations within NICE Guidance NG32.
The tool began its life with the help of guidance and support from the Rotherham Social Prescribing Service and we are pleased to share the tool freely for download below:
The user guides above have two sections. The first section is aimed at the social prescriber who will use the social prescribing tool in delivery. The second section is aimed at commissioners and providers of social prescribing services and can be utilised to establish a framework of key components, of which a social prescribing service should provide within communities.
